WELCOME TO OUR PRACTICE
Welcome to our OMS Center-- our staff appreciates the opportunity to provide you with the highest quality of oral and dental care.
Your first visit to our office will include a consultation to explain your diagnosis as well as to evaluate your treatment options. On occasion we are able to complete your procedure on the same day as your consultation, however, a more complicated medical history and/or treatment plan usually requires a follow-up appointment.
If there are no issues with your medical history or treatment plan to require a second visit, we are happy to perform your surgery on the same day.
We understand that patients do not like having to wait long periods of time to be seen by a doctor, so have provided our required forms on our website for you to download. These forms may be similar to the ones you completed at your dentist’s or doctor’s office, but we encourage you to thoroughly read and fill out these forms as accurately as possible.
Please complete the following forms prior to your appointment:
- Health Questionnaire
Please write down the names and doses of any prescription medication you may be taking and bring the information to your first consultation. This will shorten your wait time and allow us to possibly perform your surgery the same day as your first consultation.
If you will be undergoing sedation for your surgery, please remember the following:
- Do not eat or drink (even water) for six hours before your appointment
- Bring an adult with you to drive you home from your appointment
- Take prescription medicines) as directed with only a small sip of water
- Please tell your doctor if you are taking insulin or pills for diabetes.
If you plan on utilizing inhalation sedation for your surgery, do not eat more than a light meal in the two hours leading up to the surgery. Continue taking prescription medications as directed.
X-Rays and Referral Slips
If you have been referred by a dentist, please bring your referral slip and x-rays at the time of your appointment. Keep in mind that some x-rays may need to be reperformed depending on quality. Remember-- you do not have to be referred by your dentist to see our doctors; we accept patients directly as well as through referrals.
Children under the age of 18 must be accompanied by a parent or legal guardian.
While we do our best to maintain our patients’ scheduled times of appointment, we ask that you remember that oral surgery sometimes comes with unavoidable delays that can push appointment times back.
